All athletes and their guardians must review the BLS Athletic Handbook and agree to its terms.
When trying out for a sport at BLS, the following requirements must be completed prior to the first day of tryouts:
1. Sports Clearance Form - issued by the BLS Nurses when school is in session. For fall sports, students should obtain clearance forms prior to the end of the school year if possible. To get the form, a current physical (completed within 13 months of the start of the season) must be on file in the nurse's office. The nurses will only issue this form to students. If an injury or concussion has occurred after the date of the physical, an additional note clearing the student for play must also be submitted.
2. Concussion course and concussion information. All players and parents must complete an online concussion course. The course can be found here or at www.cdc.gov/concussion. This should be done before filling out the online registration (see below). Players and parents must disclose any head injuries and/or concussions where indicated on the online registration form.
3. Opiate/ Substance Abuse Education: All student-athletes and parents must read the information regarding opiate and substance abuse provided in this file labeled "Opiate Substance Information". Additional information is provided here as well. You will be required to acknowledge that you have read the information prior to registering.
4. Sign up on the BLS Sports Registration Site. Sign up for each child by clicking on the link below.
